How to Refer EMIS Forms

Urgent referrals for suspected cancer should be sent using the Pan-London suspected cancer referral forms. Please check that you are using the correct version of the form on your EMIS system. 

Send by e-Referral, not fax or email. Book directly with the preferred hospital (not via CCAS).

When referring a patient...

  • Please ensure they are aware of the urgency and reason for the referral and are available and willing to attend an appointment within two weeks
  • Patient information leaflets are available for some referral types and in 12 languages from Pan-London website
