Camden Council has commissioned Age UK Camden to support pathway 0 patients discharged from hospital following a COVID-19 admission.

Pathway 0 covers the majority of patients being discharged from hospital following a COVID-19 admission, who will be a simple discharge, with no additional ongoing health or social care needs. The Age UK Single Point of Access will provide co-ordinated rapid support tailored to individuals.

Assistance will be provided for:

  • wellbeing phone calls
  • food provision including emergency food parcels, hot meals, shopping
  • prescription delivery and collection
  • general amenities for example heating check, gas top-ups
  • information and advice including debt, benefits, housing, employment
  • mental health support

Referrals can be made into the service by any health or social care professional using either the attached form or the online referral form.


Eligibility criteria

Inclusions

  • discharged from hospital following a COVID-19-related admission
  • medically optimised 
  • 18+ 

Exclusions

  • under 18 
  • has ongoing medical needs as a result of their admission
