Camden Integrated Care Service (CICS) aims to manage co-morbidities and frail patients with specialists from different disciplines working out of the same location to better support your patients with long-term health conditions.

CICS referrals can be made for patients with:

The CICS for epilepsy, COPD and heart failure are no longer available. Referrals for epilepsy should be sent to secondary care via CCAS, unless it's first-seizure epilepsy. For COPD and heart failure, please see the information letter.

Referral methods: E-referral

Via e-Referral to CCAS:

Complete the CICS Proforma in EMIS (see EMIS Forms).

Create an e-Referral via your GP clinical system.

In e-Referral, select Specialty/Clinic Type: General Medicine, then Service: Camden Integrated Care Service (CICS).

Send UBRNs (Unique Booking Reference Numbers) to CCAS.

Referrals will then be entered onto EMIS Web by CCAS and then viewed and triaged by the respective discipline. All referrals will be reviewed by a specialist, leading to one of the following outcomes:

  • onward hospital referral
  • community assessment
  • virtual assessment with a management plan feedback to the GP
