Anticoagulation Service – UCLH

The Anticoagulation service at UCLH comprises of a nurse-led clinic supported by a multidisciplinary team. The clinic initiates and monitors patients on vitamin K antagonist (eg warfarin) and the initiation and review of patients on direct oral anticoagulants (DOAC).

Patients who have been initiated on warfarin should have been provided with an NPSA oral anticoagulant therapy record book (see Downloads) by the hospital clinic. This booklet should be taken with them to any hospital/GP appointments and when collecting anticoagulant prescriptions.
