Due to issues in the laboratory (on 20 November 2018) at Royal Free Hospital some Haematology results have been duplicated across a number of patient records – this is being investigated by HSL IT.
Royal Free Hospital is confident that all affected patient results have been identified and corrected with amended reports being sent out to the patient record. The majority of the results were corrected on 23 November 2018 and the rest will be completed on 29 November 2018. Any results within RFHs critical or urgent call ranges when amended were communicated to the respective clinical teams.
You may have received an amended report for your patient between the time period of 20 November–26 November 2018. Every care has been taken to make sure the correct results have been transmitted to the patient record.
RFH apologises that all results may not be transmitted and a few may be reported as ‘regret no result available due to technical error’. If this result is clinically indicated please send a repeat sample.
