Type 2 Diabetes and Hypertension tools now available from UCLPartners
In response to the COVID-19 pandemic there have been huge efforts across the primary care system to change ways of working in order to keep patients and staff safe.
UCLPartners have designed a series of frameworks to help primary care teams build new ways to deliver proactive care to their patients with long-term conditions to keep them well and to maximise resilience in the face of COVID-19.
New tools and resources for Type 2 Diabetes and Hypertension have been added to the resources for asthma and COPD.
The support package includes:
- search and risk stratification tools for EMIS and SystmOne
- pathways to support remote management and self-management using the wider workforce (e.g. healthcare assistants, link workers and pharmacists)
- digital resources
- training that includes motivational interviewing and health coaching
In addition to these resources, the UCLPartners in-house team are able to provide clinical and project management support for local pathway adaptation. This includes training and education for HCAs, link workers, pharmacists and others, comprised of virtual workshops, evaluation and communities of practice.
