This session is part of the Quality Improvement Programme for practice managers and staff, which will consist of six sessions. Each session will be for up to 50 delegates – attendees can pick which session (or sessions) they would like to attend. Please note, each session must be booked separately.
This workshop will cover the active signposting and social prescribing elements of the HIAs. Active signposting provides patients with a first point of contact which directs them to the most appropriate source of help.
Web and app-based portals can provide self-help and self-management resources as well as signposting to the most appropriate professional.
Social prescribing is the use of referrals and signposting to non-medical services in the community that increase well being and independence.
This session will involve some pre-work and a mapping exercise to look at what is available within the localities and examples from across the country looking at successful methods.
