CamEd March 2019 Event at the RCGP

Topics covered will include: Homelessness, End of Life care, Basic Diabetes and Non-clinical Practice Staff training.


Sessions

Homelessness — Morning

Thursday, 14th March 2019 / 9.00am

The Homelessness service under the Universal Offer aims at improving access, reducing  inequalities and providing flexible person-centered healthcare arrangements that meets the primary health needs of the vulnerable homeless population in Camden.

This is a mandatory event for the Homeless Lead Clinician to attend. Please be advised that practices are to discuss lesson learned and impact of the homeless educational event with their practice members and to then submit the action plan survey via citizen space.

There will be an opportunity to have questions answered, share concerns, ideas and experiences during the event.

Topics to be discussed will include:

  • Chronic Leg Ulcers (wound care)  
  • Substance  Misuse 
  • Groundswell Care Co-ordination 
  • Update on Network – Jasmin 
  • Hospital Discharge 
  • Barriers to access
  • No recourse to public funds
  • Case Management

 

Backfill

This backfill is for Camden Practices only
As part of the Universal offer backfill is available
1 x Lead Clinician   £250 (mandatory)
1 x Practice Nurse   £140 (optional but recommended)
Action Plan Survey £150 (submission via citizen space :deadline 10/04/2019)
Note:  No extra payment will be made should a practice send more than one representative in each category.

End of Life Care (EoLC) — Afternoon

Thursday, 14th March 2019 / 14.00pm

The End of Life Care (EoLC) service under the Universal Offer aims to ensure that patients are treated in the most appropriate environment, according to their end of life wishes where practical and  reduce, across Camden as a whole, the number of avoidable hospital admissions for patients nearing the end of life and thus reducing secondary care spend.

This is a mandatory event for the EoLC Lead Clinician to attend. There will be an opportunity to have questions answered, share concerns, ideas and experiences during the event.

Topics to be discussed will include:

  • Updated Camden CCG Palliative Care Prescribing Guidelines
  • Last Days of Life checklist and GPs, Palliative Care Teams and Community Nurses working together
  • Co-ordinate my Care – update and presentation from the CMC Team
  • Case-based discussions with specialists from the two Camden Palliative Care Teams – please bring any cases or dilemmas along for discussion
